When you submit the appointment request form, we collect the name, email address, phone number, the body area you selected and anything you write in the message field.
The site also records standard technical information such as pages visited and approximate location, used only to understand how the site is performing.
How it is used
Contact details submitted through the form are used to respond to your request and to schedule care. They are not sold, and they are not shared with advertisers.
Do not send medical details
Please do not include diagnoses, test results or other sensitive medical information in the website form. It is not a secure clinical channel. Discuss those details directly with the practice.
